Dexpanthenolis available in the market in concentration

Name

Dexpanthenol

Precaution

Dexpanthenol should be used with caution in patients with a known history of hypersensitivity to panthenol or related compounds. Careful monitoring is advised when using topical or injectable formulations near mucous membranes or broken skin due to the risk of local irritation. In rare cases, allergic reactions may occur. When administered intravenously or intramuscularly, monitor for local injection site reactions. Although generally safe, it should be used cautiously in pregnant and breastfeeding women unless clearly needed.

Indication

Prevention and treatment of pantothenic acid deficiency Post-operative paralytic ileus and abdominal distention (injectable form) Promoting wound healing in minor burns, cuts, abrasions, and skin irritations (topical form) Treatment of diaper rash, sunburn, and nipple irritation during lactation Supporting hair and skin hydration in cosmetic products

Contra indication

Patients with known hypersensitivity to dexpanthenol or any component of the formulation Severe renal impairment (injectable form), due to the risk of accumulation Hemophiliacs or those with bleeding disorders when injected intramuscularly, unless clearly indicated

Side Effect

Topical use: Local irritation, redness, burning, or contact dermatitis Parenteral use: Injection site pain, mild gastrointestinal disturbances (e.g., nausea), rare allergic reactions like urticaria or bronchospasm Systemic: Hypersensitivity reactions in rare cases, including anaphylaxis

Mode of Action

Dexpanthenol, the alcohol analog of pantothenic acid (vitamin B5), is rapidly converted into pantothenic acid in the body. Pantothenic acid is a component of coenzyme A (CoA), essential for fatty acid metabolism and cellular energy production. It promotes epithelialization, wound healing, and keratinocyte proliferation in damaged skin and mucosa.

Interaction

It may enhance the cholinergic effects when used concurrently with cholinergic agents (e.g., neostigmine), especially when used for paralytic ileus Antibiotics or antiseptics in topical formulations may alter its effect if not compatible No major systemic drug interactions have been reported

Pregnancy Category Note

Pregnancy Category: Not assigned by FDA Dexpanthenol is generally considered safe during pregnancy when used topically. For injectable forms, caution is advised, and use should be limited to situations where the benefit outweighs potential risk. Animal studies have not shown teratogenicity, but adequate human data are lacking.

Adult Dose

Parenteral (for paralytic ileus): 500 mg IM once daily Topical: Apply a thin layer 1–3 times daily to the affected area

Child Dose

Topical: Apply 1–2 times daily as needed, under pediatric supervision Parenteral (off-label use): Dose adjustments based on weight and clinical condition, not routinely recommended without medical oversight

Renal Dose

No specific adjustment required for topical use For parenteral administration, caution is advised in patients with renal impairment, especially in severe cases. Use only if clearly indicated.

Administration

Topical: Apply to clean, dry skin. Do not apply to deep or puncture wounds without medical advice. Parenteral (IM): Administer in the upper outer quadrant of the gluteal region using aseptic technique. Do not administer intravenously unless formulation specifies IV use. Avoid contact with eyes and mucous membranes when using topically.
